Reducing Healthcare Expense by Facilitating Care
In every population, healthcare costs are driven by a small percentage of that population. Individuals who have unique needs like advanced chronic conditions, acute medical situations, or are suffering from severe mental illness may account for as much as 60% of program expense, although they may only account for 10% of a state’s Medicaid population.1
APS Healthcare’s proven strategies for disease and care management define our approach for meeting complex healthcare needs: identifying the most “impactable” members of a population, eliminating barriers to receiving care, coordinating care through a highly fragmented system, and by improving the quality of life at the end-of-life.
Targeted: APS seeks to assist the most vulnerable individuals, those with unique needs requiring specialized capabilities and interventions, educating and encouraging participants to improve self-care and preventive practices, and care setting selection.
Focused: We address the hidden obstacles that stand in the way of individuals receiving appropriate care. APS personnel work diligently with local community resources to find solutions to financial, transportation, cognitive, and psychosocial barriers.
Collaborative: We partner with essential community providers, engaging them in the design and oversight of our programs as well as adding resources that extend and strengthen the quality of care delivered.
