In the United States we spend over two trillion dollars1 annually on health care, approximately 16% of the U.S. economy. However, only 55% of adults and 47% of children receive “correct” care2. For Medicaid, states must deliver the right services in the right settings, but closing the gap between evidence-based guidelines and the realities of practice in today’s fragmented healthcare environment requires attention.
Providers need integrated and organized information they can use to identify patients with gaps in care, set priorities for delivery of services, and follow patients over time to increase adherence to evidence-based plans.
