Milwaukee County is offering employees and their dependents a new health and wellness program provided in partnership with APS Healthcare. For those living with a chronic condition or illness, a new Disease Management program is offered as well.
These programs provide you with online access to valuable tools, information and services to assist you in making healthy lifestyle choices and managing your condition. The program is available at NO COST and is completely voluntary and confidential.

Milwaukee County want to support you in living a Healthy Lifestyle. The benefits of a healthy lifestyle are priceless—lower risk of disease, greater likelihood of living longer and able-bodied—just to name a few. As a part of the Healthy Together program, you can earn incentives by engaging in healthy activities and using the programs available on this site. Through a new health incentive program offered to you through all Milwaukee County plans in partnership with APS Healthcare. The Healthy Together program provides you with access to a Health Assessment, Disease Management Programs, Lifestyle Behavior Change Programs, and Online Educational Tools at no cost to you.
Instead, you can actually earn cash rewards on your paycheck. Here’s how it works:
*Take part in a Wellness Screening to be held at designated County worksites. This screening will measure your: Total Cholesterol; HDL; Blood Glucose; Blood Pressure; Weight; Height; and Waist Measurement to calculate Body Mass Index.

Health Risk Assessment (HRA)
The HRA is the first step to a healthier you and allows you to complete a comprehensive assessment of your overall health. You can complete the HRA online by clicking here.
Once you complete the HRA, you will receive a personalized wellness plan, as well as ongoing education tailored to your unique situation that will guide you in making healthy changes for life. Complete your HRA and Bio Metric Screening an earn $100 award!
Finally, submit your Wellness Screening results with your HRA and your incentive award will appear on your payroll check.
You can receive $100 for completing both the Health Risk Assessment (HRA) and a Wellness Screening. If your spouse is also participating, you can each receive the incentive.
Lifestyle Behavior Change Programs
The Healthy Together wellness program provides you with access to personalized, lifestyle behavior change programs to address specific health concerns, including smoking, stress, nutrition and weight management. Each lifestyle program includes:
- A comprehensive, condition-specific assessment
- Personalized and tailored action plan
- Follow up communications, education and support timed to address critical points in your action plan
