APS Healthcare CareConnection®

APS Public Programs provides innovative technological and managerial solutions for public healthcare programs across the country. APS CareConnection®, a proprietary Web-based technology platform, collects authorization and quality improvement data via the Intranet, and provides that data to authorized care management participants in full compliance with HIPAA requirements. Automating the review of routine, criteria compliant, authorization requests reduces the administrative burden on providers and improves quality of care.

APS CareConnection® maintains complete records of treatment history and enables APS health coaches and care managers to review an individual’s entire course of treatment, rather than an isolated authorization request, to determine the most efficient treatment plan. It also promotes effective care coordination by enabling clinicians to address treatment fragmentation when services are provided by multiple providers.

APS CareConnection® is an integrated software solution for utilization management and quality improvement. Among its unique features are:

  • Healthcare providers fill out one form, regardless of the type of service requested

  • Authorization requests previously completed may be used to complete future requests. Therefore, data entry is not duplicated. Providers simply fill in those data fields that indicate a change in the individual’s status

  • Electronic submission eliminates mistakes as the majority of data entry choices, including medication data, is made from pull-down menus

  • State and federal reporting requirements may be integrated into APS CareConnection® and made available for routine and ad hoc reports

  • Flexibility to integrate with a variety of Medicaid Management Information Systems (MMIS) platforms and associated state healthcare systems

APS CareConnection® for health and disease management

APS CareConnection® for health and disease management is the first Internet-based plan of care tool used to help coordinate care for chronically ill Medicaid beneficiaries. This technology enhances clinical care by:

  • Enabling all participants — patients, providers and health coaches — to work more effectively together using a collaborative medical record

  • Helping providers and APS health coaches monitor the health of participants in care management programs

  • Allowing providers to approve and modify their patient’s plan of care

  • Allowing program participants to self-report weight, blood pressures, blood sugar, obstacles to care and other information

  • Providing access to pharmaceutical information and enabling online e-prescribing activities
