Public Programs
APS Healthcare is one of the largest providers of specialty healthcare services in the public sector, with over 40 programs in over 20 states. APS Public Programs delivers tailored programs that are customer and program focused. We are experts at developing healthcare models and strategies that successfully address the unique medical, behavioral and psychosocial challenges faced by individuals enrolled in publicly funded programs, particularly persons with chronic diseases, developmental disabilities and severe and persistent mental illness.

Our unique decentralized operating and staffing approach is designed specifically around the needs of each customer’s program. This model places professionals directly within the communities we serve and establishes collaborative partnerships with community providers to promote coordination of care and improve beneficiary, provider and stakeholder engagement in our programs.
Information Technology
APS Public Programs is credited with pioneering innovative technology, approaches and models of care that have successfully improved health outcomes for Medicaid beneficiaries. Our proprietary, web-based care management system, APS CareConnection®, is the first of its kind in the industry. This dynamic, HIPAA compliant application offers varied capabilities based on its application to specific healthcare programs and has proven to enhance care coordination, improve the clinical validity of care plans and reduce administrative burdens for community-based providers.
Accomplishments
Since 2004, APS has provided disease management, utilization management and Quality Improvement Organization (QIO) services to Wyoming Medicaid. Program results include:
- Cost avoidance of more than $29 million for the State of Wyoming during the first two years of the program (2005 and 2006)
- Dramatic utilization and clinical improvements in each measurement year; including reducing emergency room visits by 11%, inpatient admissions by 40%, the number of inpatient days by 18% and readmissions within 30 days by 36%
APS Public Programs was recognized by DMAA: The Care Continuum Alliance (formerly the Disease Management Association of America) in 2005 and 2007 as having the best government disease management program in the United States for its programs in Wyoming and Georgia. In 2006, DMAA recognized APS Public Programs' Georgia Enhanced Care Program as having outstanding provider engagement for its partnerships with the Morehouse School of Medicine’s National Center for Primary Care and other healthcare providers in the state.
