State of Wyoming - Provider Information
The Healthy Together! program was developed by the Wyoming Department of Health to assist individuals and families covered by EqualityCare (Medicaid) insurance in realizing a healthier and more productive life. This benefit provides EqualityCare clients with 24-hour support, education, and help coordinating services with you, their provider.
The role of providers in the Healthy Together! program
Is very important; as part of the Healthy Together! team, you are able to:
- Identify, at the point of care, EqualityCare clients who will benefit from Healthy Together! program services.
- Promote the program to your patients.
- Collaborate with the Healthy Together! clinical team to create a stronger continuum of care and improved outcomes.
Healthy Together! offers Wyoming providers:
- A team of clinicians (nurses, social workers, licensed counselors, pharmacists and physicians) to help reinforce your treatment plans.
- Support from an after-hours, toll-free education line helping redirect clients away from local emergency rooms or from after-hours services.
- Educational tools and facts sheets on a variety of health and disease topics that you can use in your practice with your patients.
- Guidance on conducting group visits, a model for seeing 15-20 patients during an extended visit and producing better outcomes and increased patient satisfaction.
- Free continuing medical education on nationally recognized, evidence-based clinical guidelines for treating patients in the primary care environment.
Our goal is to support you in your care of the most fragile individuals—those who are disabled, chronically ill and elderly—with a coordinated team. The Healthy Together! care team is a crucial part of this network, and in partnership with physicians, PAs, nurses, case managers and community organizations, can make a significant impact on a person’s health and quality of life.
EqualityCare Utilization Management Program:
Healthcare professionals who provide behavioral health and select medical care also have access to the EqualityCare program’s Utilization Management (UM) program.
