Healthy Togethersm Program

Milwaukee County wants its employees to be healthy, active, and productive. But feeling your best can be difficult if you are living with a chronic health condition. That's why Milwaukee County is offering the Healthy Together program at no cost for members and their dependents enrolled in the medical plan.

The Healthy Together program is for those living with (click on a topic for more information):

The Healthy Together program is confidential. Selected individuals have an opportunity to work one-on-one with a supportive health coach. The health coach may be a specially trained registered nurse, dietitian, certified diabetes educator, licensed social worker or clinical psychologist who will provide you with information, support and guidance on ways to feel your best while living with a chronic condition. Because each member is unique, the support and education you receive is specific to your needs. Information may be available through the internet, phone, educational handbooks, newsletters, and other written materials about resources that are available to you. Additionally, your health coach can work with you to access medical and community resources.

Healthy Togethersm does not replace the care of your doctor. It is a complementary program
to the services you are already receiving and is intended only as a source of information and guidance.
Only your doctor can provide medical care for you. Contact your doctor if you have any medical problems.
 

Milwaukee County employees may be contacted on the phone or in writing by a health coach. Through a health questionnaire, you and your health coach look at how your illness affects you. Then, together, you decide what information, support or special programs would be most useful to you.

Remember, Healthy Together services are no-cost, voluntary, and confidential. Information on who is participating, as well as personal information shared with APS staff, is never shared with Milwaukee County. Cardiac

One in five women and one in five men have some form of cardiac disease. The impact of this will only increase as Americans age. Common heart conditions and risk factors include Coronary Artery Disease (CAD), Congestive Heart Failure (CHF), High Blood Pressure and High Cholesterol.  All of these conditions, like other chronic illnesses, can significantly affect a person's personal and professional life. Daily weighing, physical activity, medication, and food choices are very important to feeling your best if you have cardiac disease, high blood pressure or high cholesterol. Healthy Together understands the impact of living with an illness requiring such close monitoring and helps people understand how daily monitoring can help them feel their best and reduce their risk factor for future heart problems.

Chronic Obstructive Pulmonary Disease - COPD

Approximately 16 million people in the United States have been diagnosed with some form of COPD and as many as 16 million more may be undiagnosed. Most of these individuals suffer from a combination of diseases of the lung such as emphysema and chronic bronchitis, which cause inflammation and destruction within the lungs and make it difficult to breathe. Smoking causes up to 90 percent of COPD cases in this country. Healthy Together helps enhance the quality of life for individuals with this condition through smoking cessation, medication management, exercise, breathing exercise and oxygen therapy education and support.

Low Back Pain

Chronic Low Back Pain can happen for many reasons, including injury, faulty posture and degeneration of the different parts of the spinal column. It is an issue for many employees who need to stand on their feet for a majority of the workday.
