Instructions for Accessing the Health Risk Assessment (HRA) and Online Tools

Using the Web

  • Go to www.healthytogether.net/milco
  • If you are using this site for the first time, skip the Username and Password section.
  • Click on the Sign Up button in the New to Healthy Together? section.
  • Enter the following information:
    • First Name – Click in first name box and enter your first name.
    • Last Name – Click in last name box and enter your last name.
    • Date of Birth – Click in date of birth box and enter your date of birth. (example: 07/11/1973)
    • Member ID – Click in Member ID box and enter member id. Member ID is the employee’s social security number.  
  • Click on Sign Up and wait a few seconds or more.  Do not click the sign up button more than once or the process will start over.  When your eligibility has been verified, the program will take you to the profile screen.
  • On the Profile screen you will be asked to enter a username.
    • Your username can be anything.  Example: first initial, last name – asmith. 
    • Spouses and/or dependents should create their own personal user name.
    • Please retain for future reference.
  • Continue to fill in profile questions. 
    • You will be asked for your email address.
    • An email address is required to proceed. If you don’t have an email address, you can sign up for one at one of the many providers of free email services including Yahoo and Google.
  • You must create a password which needs to be at least 7 characters.  Please note the password is case sensitive and must contain either letters or numbers with 1 symbol (Example of a symbol: @#$%)
    • Example: aSmith#. 
    • Re-enter the same password again to confirm. 
    • Please retain for future reference
  • Complete the secret question in the event you forget your password.  The system will ask you to select a question from a drop down menu such as, your pets name, mother’s maiden name, etc.
  • Once complete Click the Update button, to continue to the Agreement screen.
  • Read the agreement and click Continue to agree to the terms.
  • Click on APS/HealthMedia (Written in Red at the bottom of the screen).
  • Click on HealthMedia Succeed. 
  • You are now ready to access the Health and Wellness Programs and the Health Risk Assessment available to Milwaukee County participants.
